Me and my wife have 2 Samsung Watch 4. Mine is 44 mm and her is 40mm.
I manage to install on both your mooded apps. On my wife is working fine for long time. On my watch, after a I'm installing the app, after a while my watch is disconnecting from phone ane I can't reconnect to my phone.
Both we have OnePlus devices. Mine is 8 and her is 7.
Do you have any idea what could lead to this?

Disconnected but you can connect again without pairing, right...?

If it's only Disconnected, I can think of one possible thing which battery optimization killing the connection, this will require you to whitelist the plugin and galaxy wearable to prevent it from being killed...

Unfortunately 90% of the time I can't reconnect without resetting the watch. 😐

Once is disconnected seems to be unpaired from Bluetooth also. Even I'm trying to reconnect it via Bluetooth and is successfully connected, still the Galaxy app can't reconnect back to watch.

Unfortunately I can't trace your issue through logs because you said it disconnects randomly and not immediately after pairing through galaxy wearable...

Bluetooth connection makes the watch release the pair keys of galaxy wearable which makes galaxy wearable unable to connect to the watch unless you reset the watch, so you must never pair through Bluetooth settings, only galaxy wearable, before pairing the watch4 through galaxy wearables, make sure you forget the watch from Bluetooth settings so your phone doesn't attempt to connect to the watch through Bluetooth settings...

You could still try the whitelist for battery optimization and see if it actually makes any difference...

i got the same Problem with similar setup. Galaxy Z Flip3 with a Galaxy Watch 4.
I installed everything the correct way, enabled bp sync but the shm does not allow me to connect with the watch as its not recognised by it but the mod app on the watch launches the shm mod.. Neither clearing every cache and data nor reinstalling helped so as of now BP and ECG are working. Maybe you got an idea.

A screenshot of the phone app isn't enough to assist at all... I really appreciate more details, like picture of the watch shm mod... even better if you can record the issue as a video...

Well the issue is that nothing is happening. No error except the message on the screen of the phone app. On the watch it just says "open phone app" And in the phone app the search function does not find the watch... I don't know anything helpful i could attach

🤦‍♂️ nvm. If get "no compatible watch" message no matter what you do, just remember: Phone goes on phone. Watch goes on watch. I know that. I KNEW that! Laughed smugly at those who installed the phone apk on the watch... yet got hasty after downloading & did it myself. *sigh*

Ok, now I'm sure you have both apps correctly installed, the next thing you can do is restart your phone and watch, then try going to shm, hopefully it works then, few reported that it works after a reboot, maybe the watch or phone is busy in background doing something which makes shm mod wait till whatever in background is done in order to connect...

I already tried this several times, also i cleared data and cache of all samsung and google services. Also by accident i did a reset on the watch and had to reinstall the mod app onto the watch.
The weird thing is that samsung health and the galaxy wearables app can communicate with the watch without a problem. Could i it be related to the root status? I am using the universal safetynet fix under magisk and the denylist to hide root from shm. I couldn't try it with the original app because it is impossible to hide a knox trip...
But the root hide works with all the other samsung apps (except main samsung health where you are stuck with an older version because of knox)
