Change number implementation in inspection plan

We have a requirement to track the changes in inspection plan (task list type "Q").
Details of requirement :
1) At creation and change of inspection, version of inspection plan should be maintained.
2) One version or change no. can only be used once.
3) System should not allow change in inspection plan which entering the version or change no.
Note ,we dont have requirement of release stratergy or workflow in our business requirement.
Please let me know what is the simplest way of implementation this requirement. I am working
with change no. for the first time, hence request you to eloborate the solution in detail.
